After filling out the application with your information, click on the "Click Here to Submit Form" button at the top of the first page. Your computer system may prompt you to select an email client. The prompt may look similar to this:

- If you use Outlook as your default email application, select Desktop Email Application then click OK. This will automatically attach the form to a new Outlook email.
- If you use a browser-based email program such as Yahoo mail or Google mail, select Internet Email then click OK. This will prompt you to save the completed document to your computer system. Open your web-based email service, create a new email message and attach the document from your computer system before sending.
- If you do not see a prompt box as shown above, simply save your completed application to your computer system, create a new email using your default email client, then attach the application to the email before sending to HResources@yourstatebank.com.
